Home Health Aides and
Personal Care Aides
Our Certified Home Care Workers are trained to provide top of the line home care services. They will assist with a variety of daily activities, this includes the following but is not limited to:
Assistance with toileting and elimination
Meal Preparation
Errands and Shopping
Dressing Assistance
Bathing Assistance
Transfer and ambulation assistance
All of North Country Home Services Home Health Aides (HHA) and Personal Care Aides (PCA) are certified by New York State Department of Health and are fully insured.

Chore and Companion Aides
Chore/Companion Workers will perform support services essential for the client’s health and welfare. These services are not intended as a “hands-on” service or physical assistance.
Our caregivers will engage your loved ones with social activities as well as cueing, prompting and supervision of daily tasks.
Some of these task include:
Light Housekeeping
Transportation
Supporting integration into the community

Nursing Services
At NCHS our nurses provide quality care in the comforts of your own home. Our RN's will provide the following services:
In-home visits to assess, direct, demonstrate and evaluate the delivery of client care.
Develop a comprehensive care plan based on a Physician prescribed plan of care for clients in their homes.
Provide training and supervision for home care workers.
Provide clinical consultation for private clients.
Coordinate plans for service with Physicians and other agencies who are providing health care.
Assess client at least once every six months, or more frequently as necessary.
Notify the client’s Physician and make any other appropriate referrals if a change in the client’s environment, informal supports, or social condition warrants.
All nurses are registered by the New York State Department of Education.
