North Country Home Services’ Home Health Aides (HHA’s) and Personal Care Aides (PCA’s) receive comprehensive classroom training and are certified by New York State Department of Health. This comprehensive training and certification prepares them to provide compassionate paraprofessional care in the comfort of your home under the supervision of our registered nurse case management team. Prior to employment our Home Health Care Workers undergo a criminal record history check and a health screening to meet New York State Department of Health regulations. North Country Home Services’ Home Health Care Workers are fully insured.

North Country Home Services offers companion services to assist clients with a variety of day to day tasks such as : shopping, doctors appointments, pet care, light housekeeping and cooking to name a few. A comprehensive list is available upon request.

Consumer Directed Personal Assistance Program (CDPAP)
North Country Home Services is a Provider Agency for Franklin County’s Consumer Directed Personal Assistance Program. The CDPAP is a New York State authorized service which permits Medicaid eligible individuals, who are self directing, to manage their own personal care needs.
The CDAP was designed for people who need personal care assistance and wish to assume responsibility for their own choices. As a consumer of the CDPAP you will control the process by identifying your needs, recruiting, hiring, and training your own personal care assistants. Personal assistance services include but are not limited to assisting the consumer with bathing, dressing, toileting, and other related personal functions, performing household chores and other activities such as nursing, transportation, and respite services that assist the Consumer to be as functional as possible within the limits of their abilities.
Your local Department of Social Services will determine the number of hours of assistance you may receive after reviewing the service recommendations provided by a physician, an assessing nurse and your local DSS caseworker.
North Country Home Services Independent Choices Program will assist you by providing you with administrative services such as processing Medicaid claims according to the hours of service authorized and provided by your personal assistant, processing payroll and benefits for your personal assistant and maintaining a confidential file on all hired personal care assistants.
